RN Direct Care Supervisor/ Geriatric Care Manager

We are looking for a full-time RN to work day shifts Monday through Friday with clients throughout King and Snohomish counties. On-Call Duties shared with RN team include a rotation of weekends and weekdays. Clients will primarily although not exclusively be in the geographic region that includes communities in King County and South Snohomish County.
DUTIES INCLUDE:
§ Conducting client assessments
§ Developing care plans
§ Nurse Delegating, supervising and training caregivers
§ Reviewing records
§ Networking on behalf of CareForce
§ Geriatric Care Management
MUST HAVE:
1. A current and valid license to practice as a Registered Nurse in the State of Washington.
2. Graduate credentials from an accredited School of Nursing; BSN preferred.
3. Two years of home health or acute care clinical experience, experience with geriatric clients a must. Experience as geriatric care manager preferred
4. Ability to read and write English proficiently, ability to communicate with patients, families, personnel.
5. A current and valid Washington State driver’s license and vehicle insurance.
6. A working vehicle to visit clients in their homes.
