About the Opportunities
Senior Internal Consultant II represents Clinical Programs (CP) strategic interests on enterprise-wide and CP cross-functional projects. The consultant is a critical thinker, problem-solver and influencer accountable for developing, planning and providing oversight for complex and large scale strategic initiatives. They serve as a clinical liaison for internal and external business partners providing consultative services on internal operations and performance of programs.
Clinical Manager is responsible for Clinical Program’s program development, oversight, evaluating effectiveness and implementation of member-centered programs focused on providing an excellent member and provider experience through delivering what the customer values. Clinical Programs Manager effectively leads change, provides oversight of operations including meeting performance guarantees, accreditation and regulatory requirements and develops associates.
Senior Clinicians provide clinical support to ensure compliance with NCQA standards, develop clinician skills in motivational interviewing and lead projects to support NCQA accreditation and Clinical Program initiatives.
Case Managers/Care Transition Managers partner with members, health care providers and member’s caregivers to identify and address clinical and non-clinical barriers to member’s optimal health. Key responsibilities include conducting relationship-based assessments, developing collaborative care plans that support member’s self-management of their conditions including pre and post discharge follow up, utilizing evidenced-based techniques to promote behavior changes and providing care coordination between providers for members with acute, chronic and/or poly-chronic conditions.
Intake Coordinator, Case Management provides outreach to members and triage into Clinical Program services, manages referrals, worked collaboratively with key stakeholders to research member related barriers, provide resources assistance to clinicians and assist with outcome reporting.
