RN Care Manager
Sounds Options is currently seeking enthusiastic, energetic and experienced RN Care Managers to join our collaborative team in Tacoma, Seattle, or the Northside area. The ideal RN can build trust and rapport with clients and their families for both long-term and short-term care management needs. Qualifications:
· Registered Nurse or BSN
· Minimum of 2 years current experience in health care; geriatric experience a plus
· Minimum of 2 years community-based health care (e.g. home health, public health, home care)
Duties and Responsibilities:
· Assess each client’s status
· Elder care coaching and collaboration with clients, families, community providers, and Sound Options' team
· Development and implementation of individually tailored plan of care
· Provide referrals to community services and medical providers
· In home client visits and accompanying to services and appointments
· Provide client advocacy for those involved with the client situation including health care providers and settings, trust departments, officers and committees, guardians, facility staff, multidisciplinary teams, schools, and other services and providers
· Assist with care cost estimation and long-term care planning
· Provide counseling, resources and education to families in crisis
· Prepare written updates for client/family
